Care Management Outreach Specialist

A company is looking for a Care Management Outreach Specialist to support enrollment in care management programs.

Key Responsibilities

Make outbound enrollment calls to members using an automated dialer

Explain the VBCM program and schedule appointments between members and Care Managers

Collect, input, and update member data in the appropriate systems

Required Qualifications

High school diploma required

Minimum of one year experience in a sales-driven, call center, or customer service environment

Experience in clinical environments preferred
