Required Qualifications .
v Must have, as a minimum, three (3) years of experience with demonstrated success in census development and occupancy management in a Skilled Nursing Facility, Assisted Living or Independent Living Senior Community, or other related health care facility.
v Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to Skilled Nursing Facilities
Major Duties and Responsibilities
Plan, develop, organize, implement, and evaluate the overall operation of the Skilled Nursing Facility under the direction of the Administrator, as well as its programs and activities, to develop census goals while also maintaining compliance with current state and federal laws and regulations.
Evaluate and subsequently adjust admission practices for the Skilled Nursing Facility as needed based on expected census goals.
Ensure there is an open and collaborative relationship between the Business Office and the Director of Admissions, working toward shared facility goals regarding occupancy and collections.
Plan, develop and implement an internal census sales strategy with Business Office Manager and Administrator to develop and maintain strong relationships with referral partners / sources such as hospitals, physicians, senior centers, department of human resources, home health agencies, hospices, etc.
Assist in developing facility collaterals in conjunction with the Administrator. Provide information to referral partners when necessary to clearly communicate the services and offerings of the facility.
Identify opportunities with new referral partners / sources and develop sales strategies to build relationships and communicate the facility’s services.
Maintain a listing of referral partners / sources, locations, and assignments with each external admissions sales team liaison.
Maintain an accurate record of available beds at all times, communication boards, room assignment listing, or other means as designated by the Administrator.
Communicate bed availability to all referral sources .
Ensure referrals are processed to include a financial evaluation of the potential resident’s payment options including but not limited to printing of the Common Working File, printing Medicaid Eligibility, and identifying the pay status with the resident / representative and Business Office prior to admitting the resident.
Stay up-to date regarding entities that require prior authorization for payment and secures prior authorization when required.
Maintain working knowledge of the facility’s practices related to all pay status types such as other insurances, Medicare A, Medicaid, Private, and hospital contracts, etc.
Understand and analyze facility fiscal health through monthly meetings with the Administrator and Business Office Manager related to net revenues and outstanding A / R as specified by the Administrator.
Ensure the admissions staff is informed of Medicaid eligibility criteria as it relates to informing the potential resident / representative of the basic information he / she will be asked to share when meeting with the facility’s Business Office Manager.
Always know the status of each referral.
Ensure residents pay status changes are accurately recorded on the facility’s internal daily census communication tool.
Ensure resident’s payor source is accurately recorded in the resident’s record at time of admission.
Ensure that behavior is aligned with facility goals for occupancy and customer service.
Ensure SNF admission paperwork is completed and filed according to facility policy.
Inform the Administrator of obstacles or barriers to admitting residents and provide solutions to such obstacles and barriers.
Ensure pertinent information is maintained daily such as referral tracking logs, anticipated discharges, potential admissions, hospital discharges and available beds listing.
Ensure that effective communication with interdisciplinary team members occurs timely regarding open beds.
Demonstrate interpersonal relationship skills by developing and maintaining trust with personnel.
Speak effectively in private and public situations and may be called upon to deliver information about the facility to various media outlets.
Ensure the daily, weekly, and monthly census reports are completed accurately.
Perform daily, weekly, and monthly census analysis reports for use by the Administrator in the facility financial planning process.
Report at least weekly and monthly on the status of census goals to the leadership team and governing body when indicated.
Participate in facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.
Collaborate with members of the interdisciplinary team, physicians, consultants, and community agencies to identify and resolve issues that improve the admissions process.
Promote teamwork, mutual respect, and effective communication.
Participate in the facility’s plan of correction response to an inspection survey and implements any follow-up as designated by the Administrator or Director of Nursing.
Assist in addressing resident complaints in the Skilled Nursing Facility.

v Establish a culture of compliance by adhering to all facility policies and procedures. Comply with standards of business conduct, and state / federal regulations and guidelines.
v Must follow safety protocols and hygiene measures to protect residents and others.
v Maintain confidentiality of protected health information, including verbal, written, and electronic communications.
v Report noncompliance with policies, procedures, regulations, or breaches of confidentiality to appropriate personnel. Report any retaliation or discrimination to HR or compliance officer.
v Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and / or administrator. Protects residents from abuse and cooperates with all investigations.
v Report any occupational exposures to blood, body fluids, infectious materials, and / or hazardous chemicals in accordance with facility policy.
v Participate in all life safety and emergency drills and all required training. Fulfil responsibilities as assigned during implementation or activation of the facility’s emergency plan.
v Report work-related injuries and illnesses immediately to supervisor.
v Follow established infection control policies and procedures.
v As a condition of employment, complete all assigned training and skills competency.
v Perform other duties as assigned.

Compliance will be a factor in evaluating job performance. Violations, including failure to report violations, will result in disciplinary action, up to and including termination.

Periodic revision may be necessary to reflect changes in expectations placed on Skilled Nursing Facilities by various governmental agencies. This job description will be reviewed and / or revised annually and as needed.
Individual performance will be evaluated using the following scale :
1. Unsatisfactory : Achieves results which are far less than the standards identified for the performance factors rated.
2. Needs Improvement : Achieves results which are less than the standards identified for the performance factors rated. Exhibits the potential to become a competent performer. May be new to job or need skill development.
3. Meets Standards : Achieves results which meet the standards identified for the performance factors rated. This rating is the expected level of performance.
4. Exceeds Standards : Achieves results which usually exceed the standards identified for the performance factors rated.
