Home Health Physical Therapist - Travel Position

Location : Fort Stockton, TX

Job Type : Travel Contract

Duration : 13 weeks

Shift : 5x7s

Pay : $2475 / week

Job Description :

We are seeking a compassionate and skilled Home Health Physical Therapist to provide rehabilitation services to patients in their homes. The PT will evaluate patient conditions, develop personalized treatment plans, and help patients improve mobility, strength, and independence. The ideal candidate is patient-focused, self-motivated, and comfortable working independently in a home health setting.

Key Responsibilities :

Conduct patient evaluations and develop individualized therapy plans.

Provide therapeutic exercises, mobility training, and functional activities.

Educate patients and caregivers on exercises, safety, and home modifications.

Monitor progress and adjust treatment plans as needed.

Document visits, assessments, and outcomes accurately in compliance with regulations.

Collaborate with physicians, nurses, and the interdisciplinary care team.

Qualifications :

Doctor of Physical Therapy (DPT) or Master’s / Bachelor’s Degree in Physical Therapy

Current state Physical Therapy license

Basic Life Support (BLS) certification, COVID vaccine, recent TB test

Previous experience in home health or rehabilitation

Valid driver’s license and reliable transportation
