Fee for Service Rates :

Initial & ROC Weekday visit rate : $100

Initial & ROC Weekend / Holiday visit rate : $120

Revisit daily rate : $85

Orientation daily rate : $250

In-Service hourly rate : $55

Weekday and weekend shifts available.

As a Community Health Nurse, you will :

• Deliver comprehensive clinical home care services, encompassing patient assessments, medication management, and patient / family education.
• Take primary responsibility for coordinating, implementing, and continuously evaluating the home care needs of your patients.
• Review patient insurance details and manage care plans while ensuring vital information is communicated with families and other caregivers.
