Experience with arranging Durable Medical Equipment, Skilled Nursing Facilities, and ambulance transport

Ability to perform tasks independently

Description :

We are seeking a dedicated Case Manager with a background in nursing.

The ideal candidate should possess acute care hospital experience specifically in Utilization Management and Care Coordination.

Candidates must be able to rotate every other weekend.

Experience with Health Maintenance Organizations and Independent Practice Associations is preferred.

This role requires strong organizational skills to develop treatment plans and coordinate care effectively.

The Case Manager will work with an interdisciplinary team that includes physical therapy, respiratory services, social services, and other key support staff.

The typical caseload includes managing the care of up to 25 patients.

Candidates should be familiar with current procedural terminology coding and documentation standards.

An emphasis will be placed on maintaining compliance with regulations from Centers for Medicare and Medicaid Services and the National Committee for Quality Assurance.

Strong communication skills are essential to ensure effective collaboration with patients and team members.

The role demands a commitment to quality improvement and adherence to evidence-based guidelines for medical necessity.

Successful applicants will demonstrate competence in concurrent review and discharge planning.
