New York Licensed Care Manager

A company is looking for a Care Manager to plan and manage behavioral and physical care for members.

Key Responsibilities

Advocates and educates members on services and self-management techniques

Conducts assessments, develops care plans, and generates referrals to appropriate services

Coordinates with healthcare teams to ensure person-centered care and optimal health outcomes

Required Qualifications

NYS RN or LCSW / LMSW (any state) for Medical Care Management

3 years of work experience in a mental / behavioral health or addictions setting for Behavioral Health Care Management

1 year of pediatric clinical field experience for PEDS positions

Licensure as LMFT, LMHC, LPC, or licensed psychologist (any state) for Behavioral Health roles

Experience managing member information in a shared network environment
