Heart Failure Nurse Practitioner - Lowell General Hospital, Lowell, MA

Lowell General Hospital  is currently seeking an outpatient  Cardiology Nurse Practitioner  to join our   Heart Failure Program team within the ConnectCare Hub in  Lowell, MA.  This part-time position will be 24 hours per week with 8-hour shifts.

This position will provide advanced clinical care to patients with heart failure in an outpatient setting, collaborating with our cardiologists, primary care physicians, nurses, and other healthcare professionals. This position will manage complex heart failure cases, optimize therapy, reduce hospital readmissions, and improve patient outcomes and quality of life.

The Heart Failure Clinic provides comprehensive and individualized disease management, education, support, and follow-up to patients with advance heart failure, with the aim to manage symptoms early and prevent avoidable hospitalization. Our cardiac nurse practitioners work to optimize and transition the patients care by intensely managing medications, coordinating care with home agencies, and use telehealth monitoring at home to pick up the earliest signs of decompensation. Our team is committed to providing immediate responses to acute concerns with telephonic medication management, clinic urgent visits inclusive of provider visit and on-site IV Lasix, and utilization of Mobile Integrated Health with paramedic visits within-home IV Lasix as needed.
