Descripción del trabajoPatient Care : Manage acute and chronic medical conditions, including but not limited to diabetes, hypertension, respiratory diseases, and neurological disorders. Prescribe appropriate medications and treatments, adjusting them as necessary based on patient response and evolving care needs. Collaboration : Participate actively in interdisciplinary team meetings to discuss patient progress, treatment plans, and discharge planning. Documentation and Compliance : Complete documentation in a timely manner, including progress notes, medication orders, and discharge summaries. Patient Education : Provide counseling and support to patients facing complex medical decisions or challenges. Quality Improvement and Training : Mentor and provide clinical guidance to nursing staff and other healthcare providers as needed. Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY. Certification as an NP or PA from an accredited program. Previous experience in geriatrics, rehabilitation, or long-term care preferred. Strong clinical assessment skills and the ability to make autonomous clinical decisions. Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team. Mobile Primary Care is an independent Medical Practice headquartered in Buffalo, NY. It was started by (3) Nurse Practitioners with a passion to keep older people in their preferred living setting until end-of-life. To that end, the Mobile Primary Care practice cares for homebound people over the age of 65 in their home, assisted living, or skilled nursing facility. Home and assisted living patients are centered primarily in Buffalo, NY. Skilled nursing facilities extend across the state of NY. Overview : We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our skilled nursing facility rehab unit and long-term care unit This position offers the opportunity to provide comprehensive medical care, manage chronic conditions, and collaborate closely with interdisciplinary teams to ensure high-quality patient outcomes in a supportive and dynamic environment.

Responsibilities & Qualifications
Conduct thorough initial assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units.
Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.
Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being.
Participate in quality improvement initiatives, clinical audits, and performance improvement projects to enhance patient care outcomes and facility practices.
