Monday, Wednesday, Friday 9a-5p

Conductfollow-up visits (in-clinic, home, or via telehealth)

Performphysical exams

Reviewmedications and new health conditions

Collaborate with physicians and other healthcare team members to ensure a smooth transition to the next phase of care

FNP, ACNP, AGACNP, and PA accepted
