Admissions Director

Hudsonwide Health Care is seeking a dynamic Admissions Director to join our 120-bed Skilled Nursing Facility located in Hyde Park, NY .

We are recently renovated and undergoing additional exciting upgrades—making this the perfect time to join a forward-moving, supportive team.

We are looking for a candidate with prior experience as a Admissions Director and strong knowledge of local hospital systems. If you thrive in a fast-paced, resident-focused environment, we want to hear from you.

Salary : Commensurate with experience

DUTIES & RESPONSIBILITIES

Coordinate the resident referral, intake, and approval process in an efficient and timely manner

Build and maintain strong relationships with local hospitals, physicians, and community organizations to support consistent admissions

Demonstrate full understanding of the admissions process, including policies, procedures, assessments, and required documentation

Support facility marketing efforts to promote a positive image and cultivate new referral sources

Maintain working knowledge of Medicare, Medicaid, and Managed Care regulations and reimbursement processes

Communicate the specific needs of new admissions to the care team to ensure a smooth transition

Schedule and conduct facility presentations, on-site tours, and family meetings

REQUIREMENTS

Prior experience as a Post-Acute or Long-Term Care Admissions Director or Admissions Coordinator

Ability to work independently and collaboratively with an interdisciplinary team

Strong communication, organization, and time-management skills

Results-driven mindset with the ability to implement and follow structured processes

Proficient computer skills; experience with MS Office required

Preferred experience with Sigma, NAVI, and Allscripts
