Role Description

This is a part-time, on-site Registered Nurse role based in Wayne, Oakland, Washtenaw, or Macomb County. The Registered Nurse will provide direct patient care, conduct health assessments, develop and implement care plans, and administer medications. The role involves close collaboration with physicians, caregivers, and other healthcare professionals to ensure a high standard of care. Regular documentation, patient education, and active participation in improving patient outcomes are key responsibilities.

Qualifications

• Strong clinical skills, including patient assessment, care planning, and medication administration
• Knowledge of home health care practices, protocols, and regulations
• Previous experience in Home Care is preferred.
• Effective and quick communication and collaboration skills to work with patients, families, and healthcare team members
• Proficiency in maintaining accurate records and documentation
• Compassionate approach to patient care and the ability to provide education and support
• Licensed as a Registered Nurse (RN) in the state of Michigan
• CPR certification required; Advanced certifications are a plus
• Experience in home health care or a related field is preferred but not required
• Ability to go to and from patient's homes
• Familiarity with Axxess or willingness to learn the software
• Ability to meet deadlines and requirements consistently
• Time management skills
