PRIDE Health is seeking a Social Work Case Management for a travel job in Bronx, New York.

Under supervision, provides social services to clients / patients, such as; recipients of public assistance, food stamps and / or medical assistance, adults receiving or needing institutional care or protective services. Identifies, develops and implements social service plans for such families / individuals – determine the eligibility for these services and may perform counselling and investigatory activity related to the work.

This is the pay range that RightSourcing (a part of Magnit) reasonably expects to pay someone for this position, however, as a supplier your expected pay range may vary and / or include certain benefits like : Medical, Dental, Vision, 401K [include any compulsory benefits such as commissions, incentive bonuses, etc. if applicable].

Pay range : (USD)17.98 hrly -(USD)30.98 hrly
