Job description

URGENT NEED. The RN Nurse Assessor will perform comprehensive in-home pre-evaluations of patients to guide individualized long-term care service plans. Responsibilities include reviewing medical documentation, documenting concerns, discussing health care options with consumers, entering evaluation data, collaborating with interdisciplinary care teams, and promoting health and illness prevention. Knowledge of general professional nursing practices, state regulations, and strong organizational and communication skills are required. All candidates must be able to work independently and in a fast-paced environment.
