Job Description

The RN Inpatient Care Manager coordinates the care and services of selected member populations to facilitate a safe and timely transition to the next appropriate level of care. This role involves clinically assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to meet individual health needs while promoting effective utilization of healthcare resources.

Responsibilities

• Develop and maintain a comprehensive case management plan of care, reviewing clinical findings and working collaboratively with physicians.
• Identify and refer high-risk patients to social services for necessary psychosocial intervention.
• Conduct assessments and implement strategies to proactively prevent readmissions while evaluating causes for any readmissions.
• Collaborate with the interdisciplinary team to develop and implement a discharge / transition plan, modifying as needed to meet patient needs.
• Monitor and communicate financial management issues that may impact post-acute care planning.
• Facilitate patient progression and ensure adherence to expected length-of-stay and clinical targets.
• Coordinate transitions of care by fostering communication among patients, families, and healthcare team members.
• Stay informed of relevant federal and state regulations, including payer rules related to case management.
• Provide education to patients / families and the healthcare team regarding effective care progression and safe transitions.
• Identify potential quality issues and work with the healthcare team to address these concerns appropriately.
