Descripción del trabajoFully remote role with 50–75% travel for face-to-face member visits Full-time, 40 hours per week Monday–Friday, 8 : 00AM–5 : 00PM EST Training conducted remotely via Microsoft Teams Candidate must reside in Miami-Dade County Bilingual (English & Spanish) fluency required Conduct face-to-face and telephonic assessments with Medicaid Long Term Care / Comprehensive Program members Develop, implement, and monitor member care plans Coordinate services such as prior authorizations, PCP communication, skilled provider coordination, medication review, and community resources Utilize critical thinking, clinical judgment, and motivational interviewing to support member engagement Perform data review and documentation using care management tools Conduct multidisciplinary reviews to optimize member outcomes Identify and escalate quality-of-care concerns Provide education, coaching, and guidance to empower members in healthcare decision-making Travel to member homes, Assisted Living Facilities, and Skilled Nursing Facilities throughout Miami-Dade County Bachelor’s degree in Social Work or related field required Case Management experience required Long Term Care experience preferred Proficiency in Microsoft Office (Excel, Teams, Outlook) Strong written and verbal communication skills Ability to multitask and adapt in a fast-paced environment Ability to work independently and maintain organization Candidate must be fully bilingual (English & Spanish) — able to speak and write fluently Valid High School Diploma or GED (verifiable) This is NOT a nursing role — no RNs or LPNs Experience in Medicaid, LTC, or community-based case management Experience conducting home visits or field-based assessments Knowledge of community resources in Miami-Dade County
