Connecticut Institute For Communities, Inc. (CIFC) / CIFC Health seeks to hire a full-time Peer Recovery Coach to work in our Behavioral Health Department.  Under the direction of the Lead BH Community Health Worker, this position is responsible for providing support and guidance to individuals seeking substance use disorder treatment. The peer recovery coach, an individual in sustained recovery, that can utilize their experiences to mentor or coach, provide and advocate effective recovery based services, Peer mentoring is inclusive of information and guidance relevant to a clients recovery needs. (social skills, Substance use recovery supports, training, mentoring, behavioral skills management, employment, and recreational activities). The peer recovery coach will assist team in : 1. supporting the clients goals for recovery 2. with interventions that will assist clients navigate their journey of recover. This position is responsible for providing supportive social services including bi-directional referrals and follow up as required in accordance with CIFC Health standard of care. And assist with financial insurance assistance to CIFC Health  patients seeking Behavioral Health / Substance Use Disorder treatments and their families, In addition, this position is responsible for supporting the CIFC Health  providers by providing assistance with substance abuse treatment support and referrals.

Essential Job Responsibilities :

  • Provide education on substance use disorder and skills training in illness and recovery management to clients and their families.
  • Ability to work collaboratively with clients to assess their hopes, strengths,      accomplishments, and challenges in regards to their recovery  goals.
  • In collaboration with other care team members, works clients to develop and assess needs and develop goals and treatment plans inclusive of the clients personal interests, goals, strengths and challenges regarding recovery.
  • Actively identify and support referral and linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, professional services, etc.) that support the clients recovery goals and interests.
  • Provide in-person and telephonic support, coaching and mentoring to help clients meet their recovery goals.
  • Providing informal counselling, discuss issues, concerns related with basic needs and community resources and facilitates peer recovery groups
  • Facilitating healthcare and social service system navigation
  • Screenings for social determinant of health
  • Determining eligibility and enrolling individuals into health insurance plans
  • Serves as point person receiving and facilitates referrals for Substance Use Disorder treatment services, in collaboration with providers. This includes referrals internal from CIFC Health department and from community and referral to withdrawal management  and inpatient substance abuse / co-occurring treatment programs
  • Responsible for collecting clinical data and records prior to intake appointment including records from other facilities, package of ratings to be completed by patients and appropriate collateral.
  • Communicates with other providers internally and externally regarding substance use disorder referral outcomes by creating Provider letters, Discharge Letters, and Summaries in cooperation with providers.
  • Participates in Clinical Team Meetings and communicates with providers involved in patients’ care to facilitate all communication and coordination with the team.
  • Attend provider team meetings to promote collaboration of service and treatment providers.
  • Attend and participate in individual and / or peer supervision as directed
  • Adheres to all HIPAA regulations, including those related to the heightened protection of health records, and maintains confidentiality at all times.
  • Fulfills all compliance and training responsibilities related to position, including compliance with and enforcement of CIFC and CIFC Health  policies and procedures.
  • Performs other related duties as assigned.

Requirements :

Successful candidates must possess a minimum High School Diploma or equivalent. (Associate / Bachelor’s Degree preferred), Lived experience and / or in recovery from Substance Use for minimum of 2 years. Certified Addictions Recovery Coach preferred or willingness to obtain.  Certified Addictions Recovery Coach status or willingness to obtain.  Community health experience and / or community resource knowledge strongly preferred.  Computer, Electronic Health records experience including using the internet. Bilingual abilities preferred.

In addition, as a federally qualified healthcare center (FQHC), this position is eligible to apply for a Loan Repayment Program through HRSA (Health Resources and Service Administration).

Compensation details : 20-24 Hourly Wage
