Washington Licensed RN Care Coordinator

A company is looking for a Virtual RN Care Coordinator.

Key Responsibilities

Coordinate patient care and create action plans for senior health patients

Manage patient calls, triage needs, and address assigned tasks from team members

Facilitate communication between patients, providers, and care teams during transitions of care

Required Qualifications

WA Licensed Registered Nurse (RN) with the ability to obtain licensure in other states

5+ years of RN experience, including at least 1 year in care coordination or case management

Experience in complex care settings, particularly with senior health and home-based care

Advanced knowledge of care management principles and chronic disease education

Ability to use motivational interviewing and patient-centered communication techniques
