License : Active Speech Therapist license required.

Must-Have : SNF experience preferred.

Description :

Ensure prescreen is filled out and attached upon submission.

Resume and license verification must be attached.

Incomplete submissions will result in candidate decline.

Covid test required 72 hours prior to start date.

Results must be issued 48 hours prior to start date.

Review the credentialing checklist prior to submission.

State license verification is required.

Three references are required.

County criminal background check for 7 years is necessary for all counties lived and worked.

Federal criminal background check for 7 years or more is necessary.

Multi-state criminal search for 7 years or more is necessary.

Nationwide sex offender search for 7 years or more is necessary.

HHS / OIG search is required.

GSA search is required.

Social security number trace is required.

1-step PPD or Quantiferon test required.

Proof of positive PPD and chest X-ray is acceptable.

SAM search is required.

State police background check is required.

Mandated reporter form is required.

Sworn statement is required.
