CHA II-Transportation Specialist (2) Two Positions
Community Health Representative Program (CHR)
Department of Tribal Health
Hourly Wage : $15.92 / Regular / Full-Time
Location : Georgeville CHR Office
The primary responsibility is to facilitate access to health care by providing transportation services for patients of all ages to medical facilities on the Yakama Reservation and its member communities, including major metropolitan cities. All transport services will be for scheduled medical / dental appointments and may include emergency phone calls. Due to the nature of work, some counseling is required, appropriate outreach conducted and all referrals handled properly. Contracts are established through direct contract from patients and / or referrals from other programs and I.H.S. Clinic personnel. (Patients of all ages are eligible for services based on need; the first resource to be used by all patients is immediate family).
Examples of Work Performed :
Transport patients to various local health clinics, therapy centers, hospitals, kidney dialysis centers and other medical facilities in major metropolitan areas.
Establish priorities in accordance with program guidelines in making daily schedule of appointments.
Complete case management with supervisor as required.
Conduct follow-up health care services with assigned patients to ensure all appointments are kept.
Counsel patients about the importance of seeking timely medical care.
Observes patient's condition and surroundings : make appropriate referrals to other health care providers as necessary or when assistance is requested by patient.
Must oversee and assist in loading and unloading vehicle for patients requiring medical equipment (wheelchairs, walkers, crutches).
Delivers medication to patients when no other resources are available.
Maintains a daily log to record patients transported, mileage, vehicle maintenance requirements and recorded time.
Provide notification to supervisor in the event of problem with patients.
Follow all public safety laws, rules and regulations and other applicable guidelines related to traffic, vehicles and passengers.
Performs other tasks as assigned. Other duties.
Knowledge, Skills and Abilities :
Knowledge of community, Yakama Nation culture and traditions.
Knowledge of available resources to assist patients.
Ability to interpret and present information for and on behalf of patients.
Ability to cope with patients in stressful and crisis situations.
Ability to lift medical equipment's (wheelchairs).
Ability to operate vehicle in all types of traffic and weather conditions.
Ability to work closely with all CHR staff to ensure a positive work environment.
Ability to establish and maintain effective working relationships with other department staff, supervisors / managers, elected officials, health care providers and the public.
Ability to exercise good judgement while traveling in adverse weather conditions and hazardous road conditions while transporting.
Ability to work independently with minimum assistance.
Ability to complete written reports and program documents as required.
Ability and willingness to become CPR / First Aid Certified, includes ability to travel and attend training as required.
Ability to demonstrate high degree of personal integrity and be able to maintain strict confidentiality. HIPPA LAW.
Minimum Requirements :
One year related responsible community service work or any experience or education which demonstrates ability to perform the work.
Required to pass a pre-employment drug test.
Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving Permit.
Must have personal vehicle and personal insurance coverage.
Must be in excellent health without any physical limitations.
Required to pass a background check.
Position may require occasional travel, weekend or evening work. Flexibility with time essential in the development and success of this program.
