What You’ll Do:

• Diagnose, treat, and manage patient care with autonomy
• Collaborate with physicians & care teams
• Educate and empower patients
• Drive quality outcomes and innovation
• Maintain sharp and timely documentation
• Float between clinics to fill in where needed
• Follow PCMH guidelines and principles, integrating services and whole person care

What You’ll Need:

• Licensed as Family Nurse Practitioner or Physician Assistant in Texas
• Master of Science in Nursing
• Certification in area of practice or eligibility for certification
• DEA license eligibility
• CPR/BLS Certified; PALS a plus!
• Bilingual in Spanish? That’s a big plus!
• Advanced clinical skills & prescriptive authority
• Compassion for all communities
• Multitasking mastery & team spirit
