A company is looking for a RN Health Coach to provide remote support to patients and serve as a liaison with healthcare providers.

Key Responsibilities

Support LPNs in managing patient panels, including medication management and adherence to care plans

Provide remote patient care and education using digital communication tools

Serve as the interface between patients and the healthcare system, including providers and pharmacies

Required Qualifications

Current active Registered Nurse license in a compact state

2+ years of clinical experience in a specialty practice or acute care setting

Bachelor's or equivalent degree required

High-speed home internet access and a quiet home office space
