We are looking for Social Workers to provide the highest quality of care for our home health patients in a professional and compassionate manner. The ideal person will be specifically trained, competent, and perform assigned functions of care to the patient under the direction and supervision of the Director of Clinical Services and Clinical Team Manager. They must also be well connected knowledgeable about community, state, and federal resources for our patients. Qualifications :

• Master's degree from a school of social work accredited by the Council on Social Work Education (MSW).
• Three to five (3-5) years' experience preferred, must be well connected in the community and an understanding of community offerings for patients
• One (1) year experience in home care preferred.
• Ability to exercise initiative and independent judgment.
• Ability to work with individuals to enlist cooperation of many people to perform / achieve a common goal.
