Caregiver Navigator for Oncology Patients

Job Summary :

We are seeking a skilled Caregiver Navigator to join our team in supporting oncology patients throughout their care continuum.

Responsibilities :

• To triage the physical, psychological and social needs of the patient and significant others, offering intervention and / or referrals as needed;
• Enable patients with information and education thus reducing a portion of the stress involved in a cancer diagnosis;
• Empower the patient to participate in their personal care process;
• Improve understanding of their decisions and treatment plans; and
• Complete the treatment through the point of survivorship planning (when applicable).

Requirements :

Education to meet certification, license or registration requirement.

Oregon Registered Nurse License upon hire.

National Provider BLS - American Heart Association upon hire.

Within 18 months of hire : OCN.

3 years Oncology experience.
