TITLE : After Hours Visit Licensed Vocational Nurse (LVN)

OFFICE LOCATION : Sunnyvale

PATIENT TERRITORY : All Services Areas

SCHEDULE : Per Diem

SHIFT : 12am-8 : 30am or 5pm-1am

The posted compensation range of $35.70 - $49.78 / Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.

POSITION SUMMARY : The Shared Services After Hours Visit LVN is responsible for providing skilled nursing visits to patients and their families as requested by Intella Triage. Documents observations and interventions

related to the patient's environment, physical, mental, emotional and social status. Communicates care

with other members of the Interdisciplinary Group (IDG) or team, including patient's physician.

AREAS OF RESPONSIBILITY :

• Provides skilled nursing observation of the patient's condition, problems, care interventions,

patient and family outcomes, and evaluation of these, and documents this in the electronic

record.

QUALIFICATIONS :
