Join a dedicated team providing intensive, goal-oriented support to individuals facing mental health crises and substance use challenges requiring crisis stabilization. This role involves close collaboration with clinical and community partners to deliver high-intensity services aimed at helping consumers maintain community tenure.

Essential Responsibilities

  • Deliver person-centered case management using proven methods to alleviate mental illness symptoms and enhance emotional and functional well-being.
  • Provide crisis stabilization services to individuals in the community or within Crisis Stabilization Units.
  • Conduct trauma-informed risk assessments under the guidance of Qualified Mental Health Professionals to address safety concerns.
  • Apply de-escalation techniques effectively in high-stress situations.
  • Collaborate regularly with community partners, caregivers, family members, physicians, and other treatment providers until crisis resolution.
  • Support clients in achieving treatment goals, coordinate with collateral contacts, provide medication education, and assist with crisis management.
  • Encourage clients to complete daily living activities and monitor safety as needed.
  • Participate in care coordination to arrange and monitor services, ensuring clients attend mental health appointments.
  • Work towards successful reintegration of clients into their homes, communities, schools, and outpatient services.
  • Flexible schedule including some nights and weekends based on community crisis needs and coordination with management.
  • Complete all required documentation accurately and promptly in accordance with agency standards.
  • Adhere to privacy and security regulations regarding Protected Health Information (PHI).
  • Maintain professional communication and relationships with clients, team members, contractors, physicians, peers, outside agencies, and the public.

Education

Bachelor's degree preferred in Social Work, Psychology, or related fields. Associate's degree with additional relevant experience totaling at least four years is also acceptable.

Experience

Preferred candidates have three to five years of professional experience in mental health services, crisis response, or related areas, including educating and teaching clients about self-care and improving daily living activities.

Requirements

  • Completion of CPST Case Management training, KHS training, and passing background checks including KBI, child and adult abuse registries, and motor vehicle screening.
  • Completion of Adult Case Management training within 60 days of employment.
  • Valid driver's license, insurance, and reliable vehicle if driving is required.
  • Skills and Abilities

    Familiarity with community mental health services, strong organizational skills, and intermediate proficiency with Microsoft Office are essential.

    Physical Requirements

    Medium work involving occasional lifting up to 50 lbs and frequent standing or walking (26-50% of the day). Accommodations may be provided as needed.
