Family Support / Bereavement Counselor

Must be a licensed Social Worker, Marriage, and family Therapist or a mental health counselor.

Preferred 1 year experience in related fields.

• Grief counseling : Assess and provide grief counseling services, including one-on-one sessions, group counseling, and follow-up.
• Community resources : Refer families to community resources and maintain of list of resources.
• Hospice staff support : Provide bereavement support to hospice staff who are grieving at work.
• Plan of care : Develop and coordinate a plan of care based on assessments of the family's need.
• Follow up with families to ensure they are coping and their needs are being met.
