About The Position

Job Title : Hospice RN, Home Health RN

Profession : Nursing

Shift : Day shift with call requirements (call 1-2 nights per week and 1-2 weekends per month)

Hours per Shift : 36

Experience : Minimum 3 years

License : Registered Nurse

Certifications : Colorado adult protective services (CAPS) check required

Must-Have : Reliable source of transportation

Description :

The person in this position is responsible for providing patient care and case management using the nursing process and the individualized Plan of Care to assigned home health and hospice patients.

Administers skilled nursing care to patients requiring intermittent professional nursing services in accordance with Department policies and procedures.

The Registered Nurse makes the initial evaluation visit and regularly re-evaluates the patient's nursing needs.

Initiates the plan of care and necessary revisions.

Furnishes services requiring substantial and specialized nursing skill.

Implements appropriate preventive and rehabilitative nursing procedures.

Prepares clinical and progress notes.

Coordinates services.

Informs physician and other personnel of changes in the patient's condition and needs.

Counsels the patient and family in meeting nursing and related needs.

Participates in in-service programs and supervises and teaches other nursing personnel.

Additional information : Rural areas may not have public transportation, requiring access to reliable transportation.

Holiday coverage is required.
