Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating physical and psychological factors, financial, social, and community resources with a focus on social issues/discharge planning for patients of TRMC.
Educational and/or Professional Qualifications:
1. Bachelors or Associates degree in a healthcare related field, Social Work, or General Family & Consumer Services degree required or a minimum of 4 years experience as a discharge planner in lieu of education.
Previous Experience:
1. 1-2 years experience relative to crisis intervention.
2. Previous experience in resource management.
3. Ability to prioritize responsibilities and use appropriate time management skills.
Licenses/Certifications:
**if bachelors degree is in Social Work, must have license as a Registered Social Worker by the State of Louisiana. If degree is in other field, no license is required.

1. Basic computer literacy/knowledge
