All credentialing documents are in the "other documents" tab. ***You can start this process before your bid is accepted*** Provider must be contracted with the agency prior to placing a bid. ***PLEASE NOTE: There will be no OT rate for this assignment request*** An active license IS mandatory for every bid. Only list the license number in the field, not an IMLC letter number. Please pay attention to the date requested and do not add bids with an earlier date. Please do not reach out to the MSO directly. Please see your contacts in the bid. Provider must live 90+ miles from facility. Licensure: You must have an active NM license or reactivating one. Currently, obtaining a license takes 6 months if the applicant is new to practice; otherwise, it takes approximately 3 months. ** For COI's, providers must be enrolled in the NM Patient Compensation Fund (PCF). ***provider cannot be within 90 miles of the facility*** The provider must contract with the agency before placing a bid. Please pay attention to the date requested and do not add bids with an earlier date. Provider must be willing to work weekends, and take call. Must be board-certified with at least one year of experience. ACLS, PALS, AND BLS are required. Must be comfortable practicing independently and performing regional blocks. ***TRAINING FOR BLOCKS WILL NOT BE PROVIDED, AND THEY MUST PROVIDE CASE LOGS OF BLOCKS PERFORMED. *** The presentation must explain any gaps in service, malpractice claims, or sanctions from a board of nursing. **IMPORTANT NOTICE** All providers are required to be credentialed and enrolled prior to the start-date so please refer to the Other Details tab on this Assignment Request for the Facility Checklist. The Checklist is also available under the Help Tab (?) at the top right of the Locumsmart Webpage. Help > Documentation > CHS (Legacy APP) Enrollment Documents Information/Checklist. Please note in addition to the document checklist the CAQH Questionnaire also needs to be completed by all providers.
