About the Role :

In this position, you'll provide in-home primary care to patients across your region, including :

Conducting comprehensive assessments and developing care plans

Managing chronic conditions and performing routine medical procedures

Collaborating with interdisciplinary teams to ensure high-quality outcomes

Building meaningful relationships with patients and families

Travel Requirement :

This is a travel-based position that requires you to be in the Chico area on a weekly to bi-weekly basis. A travel stipend will be provided for visits to this region. When not in Chico, you'll serve patients in surrounding areas based on need and availability.

Pay : $150,000.00 – $300,000.00 per year
