[Location] : Wilmington, Women’s Housing Office

[Role Type] : Full-time - second shift (12 : 30pm - 9pm)

[Remote / In-Office Policy] : On-site

Program Description : The Transitional Housing Program provides subsidized housing and individual case management for individuals recovering from substance abuse, domestic violence, incarceration, or human trafficking.

Job Description : The program aims to help participants develop life skills, work experience, and confidence to achieve self-sufficient housing. The case manager role involves direct interaction with program partipants, helping them navigate resources, build life skills, and work toward independent living goals. This role reports to the Program Manager.

Duties and Responsibilities :

Client Support & Case Management :

  • Provide one-on-one support to program participants, helping them address their unique needs related to housing, employment, health, and social services.
  • Develop personalized service plans in collaboration with program participants to achieve their goals (e.g., finding permanent housing, securing employment, accessing healthcare).
  • Assist program participants in navigating and accessing community resources, including government programs, healthcare, food services, and other support services.
  • Maintain regular check-ins with program participants to assess progress, provide support, and adjust plans as necessary.

Life Skills & Education Support

  • Facilitate or coordinate life skills workshops to help program participants build the skills needed for independent living.
  • Provide support for program participants in finding and maintaining stable employment, including job search assistance, resume building, and interview preparation.
  • Assist program participants in setting and achieving goals related to education, employment, and personal development.
  • Crisis Intervention & Conflict Resolution

  • Provide crisis intervention and de-escalation techniques when program participants face difficult emotional or behavioral situations.
  • Act as a mediator in conflicts between program participants, ensuring a safe and supportive environment for all.
  • Documentation & Record-Keeping

  • Maintain accurate and up-to-date program participant records, including case notes, progress reports, and other documentation required for reporting and compliance purposes.
  • Prepare and submit regular reports on program participant progress, program outcomes, and other key performance indicators.
  • Ensure confidentiality of all program participant information, adhering to privacy and ethical standards.
  • Collaboration & Community Outreach :

  • Work closely with other program staff, social workers, and external partners to ensure a holistic approach to client care.
  • Build and maintain partnerships with local organizations, government agencies, and community resources to provide comprehensive services to residents.
  • Participate in community outreach efforts to increase awareness of the transitional housing program and its services.
  • Required Experience / Skills :

  • Strong interpersonal and communication skills, with the ability to build trust and rapport with program participants from diverse backgrounds.
  • Knowledge of community resources and government programs for individuals experiencing homelessness.
  • Conflict resolution and crisis intervention skills.
  • Ability to work independently and as part of a team in a fast-paced, dynamic environment.
  • Organizational skills, with the ability to manage multiple cases and priorities effectively.
  • Ability to lift or move items, conduct home visits, or assist with other on-site needs.
  • Nice-to-Haves :

  • A degree in Social Work, Psychology, Human Services, or a related field is preferred.
