Key Responsibilities

• Conduct comprehensive clinical social work evaluations for clients, assessing their psychosocial needs, strengths, and weaknesses to develop tailored treatment plans.
• Provide ongoing counseling services, utilizing evidence-based therapeutic techniques to support clients in managing their mental health issues effectively.
• Implement medication monitoring protocols under the direction of a psychiatrist, ensuring clients adhere to their treatment regimens while identifying and addressing potential side effects.
• Manage an ongoing caseload, maintaining thorough and timely documentation that complies with the organization’s policies, including progress notes, treatment plans, and discharge summaries.
• Engage in crisis intervention when necessary, employing specialized techniques to stabilize clients in moments of acute distress and facilitate appropriate referrals.
• Participate actively in multidisciplinary treatment team meetings, collaborating with medical professionals to coordinate care strategies and provide holistic support to clients.
• Research community resources and advocate for clients’ needs, assisting them in accessing essential services such as housing, food assistance, and healthcare.
• Develop and implement aftercare plans that foster successful transitions and continuity of care post-discharge from services.

Patient Population / Client Focus

The Licensed Clinical Social Worker will engage with a diverse client population, including individuals coping with anxiety, depression, trauma, and co-occurring disorders. This role is critical in providing culturally competent care, understanding the unique challenges faced by various communities in Upper Darby, and addressing systemic barriers to mental health services.

Qualifications

• A Master’s Degree in Social Work from an accredited institution is required.
• Active and unrestricted Clinical Social Worker License (LCSW) in the state of Pennsylvania.
• A minimum of two years of experience in a behavioral health setting, specifically within case management or direct clinical services.
• Strong clinical skills with proven abilities to perform psychosocial assessments, handle crisis situations, and provide empathetic support to clients.

Compensation and Benefits

The annual salary for this position ranges from USD 120,000 to 130,000, commensurate with experience.
