Licensed Practical Nurse, LPN - Home Health

PRN / Per Diem | Winter Haven, FL and Davenport, FL area

Trilogy Home Healthcare , a Humana company, is hiring a dedicated and compassionate Licensed Practical Nurse, LPN to join our award-winning team.   • Provide high-quality, compassionate care to home health patients according to patients plan of care
  • Use assessment tools to manage medical needs and coordinate with other disciplines
  • Serve as a patient advocate, communicating with physicians, clinical managers, and RNs
  • Make a meaningful impact on patients' lives every day

    • What You Bring :

  • Graduate of an accredited school of nursing
  • Active LPN license in Florida
  • Valid Florida drivers license, active vehicle insurance, and clear MVR
  • Minimum 1 year nursing experience required; Home Health experience is a plus
  • IV certification preferred
  • Strong initiative and independent judgement
