Hospice RN Responsibilities :

• Deliver skilled, compassionate care to terminally ill patients and their families
• Serve as a Case Manager and collaborate with the Interdisciplinary Team to establish and manage care plans
• Assess and manage patient comfort, including pain management and emotional support
• Provide education and emotional support to both patients and families during this difficult time
• Coordinate hospice services across all settings to ensure quality patient care
• Communicate with physicians and other healthcare providers as needed
