Senior RN Care Manager (LTSS / UAS Assessor)

Location : Albany, NY or Rensselaer, NY (Field-Based)

Compensation : Up to $63.79 / hr + 8% annual bonus

About the Opportunity

Join a leading national healthcare organization that’s transforming care delivery–one person at a time. We’re seeking Registered Nurses (RNs) with strong UAS assessment and case management experience to support long-term care members in Albany and Rensselaer Counties.

Key Responsibilities

  • Conduct 2–4 UAS assessments / reassessments daily for long-term care members.
  • Develop and manage individualized care plans, coordinating across providers and community resources.
  • Monitor member progress, adjust care plans as needed, and address barriers to quality care.
  • Educate members and caregivers on benefits, services, and disease management.
  • Document all assessments and interventions in compliance with state and federal standards.

Qualifications

  • Active NY RN license (compact not accepted)
  • Residence in Albany or Rensselaer County
  • 4+ years of RN experience; prior UAS assessment and care management required
  • Valid driver’s license and reliable transportation (public transportation considered)
  • BSN and / or Managed Care Organization experience preferred
