About the Role :

You’ll provide one-on-one, in-home care to adults with neurological and mobility challenges. These are patients who are not homebound, but prefer the convenience of receiving therapy at their residence.

What are my responsibilities?

• Identify and meet patients goals and needs
• Develop care plans using a variety of treatment techniques
• Provide quality, personalized and evidence-based care and proven interventions
• Manage your own schedule and varying clientele
• Document patient care services
