COMMUNICATION TEAM WORK CONDUCT : Communicates treatment issues and medication concerns to the staff nurse and or physician daily, and or as needed. Participates in treatment teams with other staff persons and helps write treatment plan reviews in conjunction with treatment team members. Attends staff meetings as needed. Establishes and maintains effective working relationships with managers, supervisors, coworkers, and staff of other departments. Performs other duties needed to maintain safe and therapeutic clinical environment. Demonstrates conduct in keeping with Tanner Medical Center's ethical standards. Errors may be serious and may involve injury to patient, staff, or self. Works constantly with confidential information and must constantly make decisions concerning true emotional status of patients, i.e., ability to harm self or others. Requires frequent contact with others to do job. Involves daily interactions with psychiatrist nurses, social services, occupational therapist and patient to provide appropriate services and programs to patient. Will also involve contact with family and outside agencies for patient care on a daily basis.DOCUMENTATION AND MANAGEMENT OF THE MEDICAL RECORD : Completes appropriate documentation - treatment plans, assessments, progress notes, weekly updates, and group notes in specified time limits. Documents calls and related information on designated forms. Participates in discharge planning, referral contacts, and verifies follow through and documentation of discharge plans. Checks and audits charts for documentation and or charge errors. Assist interns with documentation as needed. Assists in taking daily attendance of program patients and calls parents guardians if patients are absent and document this in the clinical record.PARTICIPATION IN THE GENERAL OPERATION OF DEPARTMENT : Responsibly manages resources in a conscientious manner. Assist in maintaining a clean and neat physical environment on the unit, including cleaning storage areas, stocking storage areas with supplies, cleaning and maintaining group rooms, in compliance with infection prevention standards. Seeks and identifies opportunities to improve care, patient flow or utilization of resources. Participates in process improvement initiatives for the department and facility as directed. Participates in the orientation and supervision of new employees and students. Assist in orienting new staff to PHP including rules of PHP and interaction with patients. Assist marketing in promoting for PHP by representing Willowbrooke at school career days and other marketing venues. Participates in training and development activities to enhance own knowledge and skills.PROVISION OF CARE : Responsible daily for the coordination of patient care through the following duties : Facilitates group therapy, identify a healthy recovery environment, determine risk for relapse, facilitate recovery education, facilitate coping skills, and or develops programming and therapeutic treatment for identified patients. Supervises patients during program hours. (Monitors hallways, timeout, reflection areas, and monitors patients during meal times and break times). Establishes and maintains positive relationships with patients, physicians, allied health professionals, and all referral sources. Works cooperatively with Program Manager in program development. Assists in meal preparation and or service with patients. Supervises patients as they enter and leave PHP and get on off transportation vans. Assists in supervision with patients as needed and provides one-on-one interventions with patients to de-escalate behaviors. Performs therapeutic holds on patients as needed. Attends debriefings on patients as needed.STANDARDS POLICY AND PROCEDURE : Stays abreast of organization's programs and services. Supports organization's customer service philosophy. Responsible for completion of required facility and departmental education under the direction of the department manager. Complies with other applicable department and or organization standards and policies. Complies with facility safety, infection control, and security program.
