Car provided by Sanford for use during working hours.\n\n-Development : \nGrow and hone your inpatient nursing skills by applying them differently than we ever have with this exciting new initiative.\n\nWhat you will do : \n\nUtilize Medical Surgical nursing skills to care for a select group of inpatient status patients in the patient's home setting utilizing remote technology and innovative tools to accomplish your nursing care.\n\nThis is a dual role position with opportunity to work as the hospital at home nurse and the command center nurse\n\nHospital at Home Nurse Responsibilities : \n\n-12 hr day shifts rotating weekend and holidays / On-call responsibility for overnight hours\n\n-Visit patients in their home setting and complete nursing assessments, education, medication administration, lab draws, admission / discharges, etc. on up to 5 patients during day time hours\n\n-1 in home visit per patient per day (minimum)\n\nSanford car will be provided\n\n-Command Center Nurse : \n\n-12 hr day and / or night shifts, rotating weekends and holidays\n\n-Responsible for triaging patient calls\n\n-Activate HaH nurse to the patient home\n\n-Coordinate remote visits for the interdisciplinary team\n\n-Work in our remote hub located at SMCF\n\nWhat you will need : \n\n-ASN or BSN Degree in Nursing\n\n-Active Registered Nurse License in North Dakota & Minnesota\n\n-Active Driver's License\n\nJob Summary\n\nThe Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes / planning, implementation and evaluation) to provide individualized nursing care to patients. Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise / knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting.\n\nCares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients move along the continuum of care. Responsible for the planning and coordination of care, patient assessment, patient education, triage, and various other nursing interventions.
