Social Services Assistant

Duties :

• Collaborate with patients / residents and their families to provide support and resource information.
• Respond to issues identified by patients / residents and their families.
• Facilitate patient / resident transfer within the nursing center to ensure a seamless transition and adjustment.
• Document patient / resident transfers and adjustments accordingly.
• Participate in the health care decision-making process under the direction of the Social Services Director
• Assist residents to and from needed outside physician appointments.
