Position : Registered Nurse – Home Health Case Manager

Setting : Home Health / Case Management

Employment Type : Travel Nursing Contract

Start Date : November 17, 2025

End Date : February 14, 2026

Duration : 13 weeks

Schedule : Monday – Friday, 8 : 00am – 4 : 30pm (8-hour day shifts), weekends as needed

Hours per Week : 40

Estimated Weekly Salary Range : $2,250 - $2,310

Registered Nurse Qualifications

Active Registered Nurse (RN) license in New York State

BLS certification (any provider accepted)

Minimum 1 year of clinical nursing experience in a hospital or acute care setting

Certified Home Health Care experience required

OASIS assessment experience required

Telemetry experience required

Experience with EPIC or Netsmart EMR preferred

Current NYS driver’s license

Strong clinical assessment, critical thinking, and communication skills

Ability to work independently, demonstrate initiative, and adapt to change in a fast-paced healthcare environment

Home Health RN Responsibilities

Develop and revise individualized care plans and discharge plans in collaboration with patients, families, physicians, and the interdisciplinary care team

Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, and discharges within agency-defined timeframes

Process medical orders, communicate with physicians regarding patient needs or changes, and complete certification / recertification orders and discharge summaries

Deliver specialized nursing care, preventive and rehabilitative strategies, and patient / family education on disease management and self-care in the home setting

Supervise LPNs and ensure compliance with care plans and patient safety standards

Maintain accurate and timely clinical documentation in compliance with New York State and federal regulations

Participate in patient care conferences, quality improvement initiatives, and peer reviews

Uphold patient privacy, confidentiality, and property rights

Demonstrate flexibility and willingness to visit patients as needed throughout Webster and the greater Rochester, NY area

Support the agency’s care management philosophy and contribute to operational and financial objectives
