    • Major Responsibilities :

  • Conduct thorough speech and language evaluations and develop personalized care plans and treatment goals.
  • Assess and treat patients with speech, language, voice, and swallowing disorders as prescribed by a physician.
  • Regularly communicate with physicians to update treatment plans and address any ongoing patient issues.
  • Maintain accurate clinical records according to state and federal regulations.
  • Manage a caseload effectively, coordinating services and advocating for optimal patient outcomes.
  • Conduct ongoing assessments and modify care plans in response to patient progress.
  • Educate patients and their caregivers on therapy procedures and supportive activities.
  • Oversee patient discharge planning, including providing home programs and referrals for community services.
  • Demonstrate age-specific competency in care provision across the lifespan.

    • Licensure, Registration, and / or Certification Required :

  • Speech-Language Pathology license issued by the state of Wisconsin
  • Basic Life Support (BLS) certification from the American Heart Association (to be obtained within 6 months)
  • A valid driver's license

    • Education Required :

  • Master's Degree in Speech Pathology

    • Experience Required :

  • Typically 3 years of experience in speech therapy in an acute hospital or home health care setting

    • Knowledge, Skills & Abilities Required :

  • Proficiency in clinical skills, with the ability to work independently
  • Intermediate computer skills
  • Exceptional communication and interpersonal skills
  • Strong organizational, decision-making, and problem-solving abilities

    • Physical Requirements and Working Conditions :

  • Must have reliable transportation for home visits
  • Able to perform manual tasks and operate medical equipment
  • Physically agile to navigate various environments and assist patients as needed
  • Capable of lifting and supporting patients up to 50 pounds
  • Ability to communicate clearly and effectively
