About The Position

Job Title : RN Home Health

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.

License : Must have a valid license in hand.

Certifications : CPR, BLS

Must-Have :

Must be proficient in OASIS and have used it within the last 6 months, or be willing to complete a retraining course.

Must be proficient in HomeCare HomeBase and have used it within the last 6 months, or be willing to complete a retraining course.

Must have reliable transportation, a current driver’s license, and personal liability insurance.

Must understand that this is a home health assignment and will involve driving to patients' homes daily.

Must accept all patients on the schedule regardless of their distance from previous appointments.

Must complete all documentation within 24 hours of the patient appointment.

Must understand there are no guaranteed hours with this contract.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures.

Responsible for the competent application of the nursing process.

Reports to the Clinical Manager.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Conducts home visits and attends periodic office meetings.

Travel will be up to 75 miles from the home clinic.

Daily patient visits will average between 5–8.

Daily productivity expectation is 30 points per week, with each visit and activity (such as documentation and travel) holding different point values.

Types of cases include start of care, wound care, and follow-up visits.

Weekend requirements vary based on location and profession.

On-call requirements also vary; please discuss during the interview.

Plan to travel approximately 30-40 miles per day.

Will visit both patient homes and care facilities.

This is primarily a contract position, with the potential for hire at the end of the contract.

Must be comfortable with traveling to various locations daily.

Local candidates will be accepted at the same rate as travelers.
