Job Title : Nurse Practitioner – Long-Term Care (Full-Time)
Position Overview
We are seeking a dedicated, autonomous Nurse Practitioner (NP) to deliver high-quality primary and post-acute care to residents in our long-term-care continuum—Independent Living (IL), Assisted Living (AL), Short-Term Rehab, and Skilled Nursing / LTC units. The NP will manage Annual Wellness Visits, acute / problem visits, Chronic Care Management (CCM), and Transitional Care Management (TCM) encounters.
Key Responsibilities
Perform Annual Wellness Visits and comprehensive intake assessments.
Manage acute / sick visits and coordinate timely diagnostics, referrals, or transfers.
Provide Chronic Care Management (CCM) —including medication optimization, goal setting, and interdisciplinary care-plan updates.
Conduct Transitional Care Management (TCM) follow-ups within CMS timeframes for residents discharged from hospitals or higher-acuity units.
Round daily on assigned caseload; document encounters in the facility EMR (PointClickCare / AdvancedMD).
Collaborate with attending physicians, therapy, nursing, and social-services teams to improve outcomes and reduce avoidable readmissions.
Educate staff, residents, and families on wellness, prevention, and disease management.
Participate in facility quality-improvement projects and regulatory surveys.
Call rotation : every 4th weekend (phone only; no on-site obligation unless clinically indicated).
Schedule
Monday–Friday, daytime rounds (8 AM – 5 PM typical)
Every 4th weekend call coverage

Qualifications
Current, unrestricted Pennsylvania RN & CRNP licenses with Prescriptive Authority
National NP certification (AGPCNP, FNP, or AGACNP acceptable)
Minimum 1 year experience in geriatrics, SNF / LTC, or hospitalist medicine preferred
Proficiency with EMR documentation and Medicare regulations (AWV, CCM, TCM billing)
Excellent clinical judgment, communication, and team-leadership skills
COVID-19 vaccination per CMS guidelines
