About The Position

We are seeking a skilled and

compassionate Nurse Practitioner or Physician Assistant to join our growing team in Brooklyn, NY. This

full-time position offers a dynamic work environment, competitive compensation,

geriatric community.

Position Details :

• Schedule : Monday–Friday
• No nights, no weekends

Small patient panel : Daily

census of 12–16 patients

Emphasis on quality care

over volume

No lengthy credentialing

process

Key Responsibilities :

Provide primary and

preventive care services to elderly patients in an outpatient setting

Conduct physical exams,

diagnose and treat acute and chronic conditions

Order and interpret

diagnostic tests, prescribe medications, and manage treatment plans

Educate patients on health

maintenance and disease prevention

Collaborate with physicians

and other healthcare providers to coordinate care for PACE Participants

Maintain accurate,

up-to-date patient records in the electronic medical record system

Requirements :

National board certification

(FNP, ANP, AGNP or NCCPA)

At least 1–2 years of

clinical experience preferred (new graduates with strong clinical rotations may
