Registered Dietitian – Garden Care Center (Franklin Square, NY)

Garden Care Center, a skilled nursing and rehabilitation facility in Franklin Square, is seeking a Redistered Dietitian to join our team.

Position Overview :

The Clinical Dietitian plays a vital role in supporting residents’ health and well-being by monitoring weight trends, preventing weight loss, and developing individualized nutrition care plans. This role works closely with physicians, nurses, and the interdisciplinary team to ensure each resident’s dietary needs are met.

Key Responsibilities :

Monitor residents’ weight changes and provide timely interventions

Complete nutritional assessments and develop individualized care plans

Recommend therapeutic diets and coordinate with the care team on special needs

Document nutritional status and progress in compliance with regulations

Educate residents, families, and staff on nutrition-related concerns

Participate in care plan meetings and quality improvement initiatives

Qualifications :

Bachelor’s Degree in Dietetics (required)

Current NY State Dietitian license (required)

Long-term care or clinical nutrition experience preferred

Strong assessment, documentation, and communication skills
