Job Summary

• Provide comprehensive patient assessments and recognize subtle changes in patient conditions.
• Collaborate with patients, families, and the healthcare team to develop individualized plans of care.
• Continuously update and communicate care plans based on patient progress and needs.
• Serve as an advocate for patients, ensuring their cultural, spiritual, and personal beliefs are respected in care plans.
• Create discharge plans that facilitate smooth transitions in care.
