Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!

Primary Job Responsibilities :

Assess a persons condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.

Collaborate with interdisciplinary team and colleagues to ensure a persons needs are met.

Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.

Utilize a variety of therapeutic modalities to maximize care provided to clients

Requirements : Vermont PT licensure

Home Health experience and OASIS experience a must as this candidate will case manage

McKesson / Netsmart EMR experience strongly preferred

Catalytic Solutions Job ID #4-291356. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
