Certifications : EPIC and Allscripts experience preferred

Description : This is a Monday to Friday, 40-hour position. Case Managers work either 7 : 00 AM to 3 : 30 PM or 8 : 00 AM to 4 : 30 PM. Case Managers are responsible for covering all units except psychiatric and women and infants units. Tasks include completing assessments on all admitted patients, attending interdisciplinary rounds, and initiating referrals for home health, DME, skilled nursing facilities, LTACHS, and IRFS. Candidates should have experience with EPIC and Allscripts. The average daily census is 1000+ hospital beds. The position requires working every other weekend and alternating holidays, covering 2 out of the 3 major holidays (Thanksgiving, Christmas, and New Year's Day). Orientation comprises 40 hours. Prospective candidates should have had academic experience in similar environments. The role may involve floating within the hospital or to nearby affiliated hospitals within the same service line. All staff will float within the service line. Return staff who previously worked must have been away for one year before rejoining as travelers.
