This role involves providing skilled nursing care in patients' homes, managing OASIS documentation, and collaborating with a multidisciplinary team to ensure optimal patient outcomes.

Key Responsibilities :

• Conduct in-home patient assessments and create individualized care plans
• Provide wound care, medication management, and patient education
• Complete OASIS documentation accurately and timely (Homecare Homebase experience preferred)
• Educate patients and families on disease management and treatment adherence
• Work independently while coordinating with healthcare providers
