Descripción del trabajoThe team consists of 1 physician and 2 advanced practice providers. The team covers general family medicine cases including pediatrics, adult and geriatrics, chronic disease management and optional OB. 115,000 square-foot facility, sitting on a 66-acre campus that includes a ¾ mile health walk and meditation garden Beds : 31 Home to an extensive array of inpatient and outpatient services , including a full-service 24 / 7 emergency department, surgery center, imaging and laboratory, sleep lab, cardiac and pulmonary rehabilitation, physical, occupational, speech and numerous physician clinics Proudly committed to bringing the highest quality of care to northeast Indiana and northwest Ohio Region's largest employer with over 16,000 employees Health system is comprised of more than 1,100 world-class providers in more than 45 specialties in over 300 locations. Specifics of the Role
- Office Hours : Monday – Friday, 8am – 5pm but potential for extended hours for improved patient access
- Minimum Required Hours Per Week : 36 hours
- Call Schedule : 1 : 8 for outpatient call
All calls are triaged through the nurses
